The employee(s) were observed wearing cotton T-shirts with no flame retardant rating. The employee(s) were exposed to the hazard of being struck by molten metal.

Bell grinding machine, the employer failed to ensure that a locking device was installed on the disconnect before employee changed out the grinding belt. Employee was exposed to the hazard of being caught in the rotating grinding belt from an unexpected startup of the grinder.

Employee(s) exposed to the hazard from being struck by an unguarded blade.     b) On or about October 22, 2014, building #2, the chuck on the Delta drill press was not guarded. Employee(s) exposed to the hazard of being caught in the rotating chuck.       c) On or about October 22, 2014, in building 1, finishing department, the employer failed to ensure that rotating belt and pulleys on the grinder was guarded. Employees exposed to the hazard of being caught in an in-going nip point.      d) On or about October 22, 2014, building 2, finishing department, the employer failed to ensure that the rotating belt and pulleys on the grinder were guarded. Employees exposed to the hazard of being caught in an in-going nip point.       e) On or about October 22, 2014, in building 2, finishing department, the employer failed to ensure that the rotating belt and pulleys on the Burr King grinder were guarded. Employees exposed to the hazard of being caught in an in-going nip point.
