29 CFR 1926.351(b)(4): Welding cable(s) in use were in need of repair:  Worksite located at 4836 Market Place Blvd. Shawnee, OK 74804. On or about October 21, 2014, and at times prior thereto, the electrode cable connected to the Miller welding machine were used when insulation was damaged and bare conductors were evident.  The employees using the welding equipment were exposed to electrical shock.

29 CFR 1926.351(b)(3): When it became necessary to connect or splice lengths of cable one to another, substantial insulated connectors of a capacity at least equivalent to that of the cable were not used:  Worksite located at 4836 Market Place Blvd. Shawnee, OK 74804. On or about October 21, 2014, and at times prior thereto, the work cable was spliced and not insulated to a capacity at least equivalent to that of the cable used. The employee using the welding cable was exposed to a hazard of electrical shock.
