29 CFR 1910.179(j)(3)(iv): Worn, cracked or distorted parts such as pins, bearings, shafts, gears, rollers, locking and clamping devices:  a) Near the loading and unloading area, On or about February 27, 2015, and at times prior thereto, employees were exposed to the hazards of stuck-by while lifting material such as but not limited to, metal beams weighing approximately 2500 lbs. measuring approximately 60 feet in length by 12 in height by 30 inches in width with lifting hooks that had a damaged locking safety latch.  b) Near the center area of the manufacturing shop in bay #4,  On or about February 27, 2015, and at times prior thereto, employees were exposed to the hazards of stuck-by while lifting material such as, but not limited to, metal beams weighing approximately 2500 lbs. measuring approximately 60 feet in length by 12 in height by 30 inches in width with lifting hooks that had a damaged locking safety latch.

29 CFR 1910.253(b)(2)(iv): Valve protection caps, where cylinders were designed to accept caps, were not always in place, hand-tight, except when cylinders were in use or connected for use:  Along the center area of the welding production bay #4, on or about February 27, 2015, and at times prior thereto, employees were exposed to the hazards of struck by when a compressed gas cylinder containing propylene gas that is used in welding operations was not connected for use and did not have a protective cap in place.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:   Along the north side of the manufacturing floor area, on or about February 27, 2015, and at times prior thereto, employees were exposed to electrical shock hazards of 120 volts from unused openings along the bottom and sides of a Square D electrical distribution panel box cat. no. QOC412L that were not effectively closed that power electrical equipment system such as but not limited to stationary tool equipment, welding generators and lighting systems.
