29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph:       a)  On October 21 and 22, 2014, employees were exposed to electrical hazards where the Cutler-Hammer Heavy Duty HVAC Safety Switches did not have covers exposing employees to accidental contact with live parts.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.552(a)(1):  The employer shall comply with the manufacturer's specifications and limitations applicable to the operation of all hoists and elevators:      a)  On October 21, 2014, employees were provided and allowed to use a Panther Products Corporation, Hand Powered Hoist, that was not equipped with approved counterweights.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
