29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: The employer does not ensure that employees required to perform servicing and maintenance on machines lockout or tagout energy isolating devices. This violation occurred on or about October 21, 2014, and at times prior thereto, in the cooking tank area where employees were exposed to thermal heat, caught-in and struck-by hazards while performing maintenance and servicing work on cooking tanks without locking or tagging out energy isolating devices. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure all energy isolating devices are locked or tagged out on all machines before employees are allowed to perform servicing or maintenance work on the machines.

29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices: The employer does not ensure that all residual energy is rendered safe before allowing employees to perform service and maintenance on machines. This violation occurred on or about October 21, 2014, and at times prior thereto, in the cooking tank area where employees were exposed to thermal burn hazards while performing maintenance and servicing work on machinery without first ensuring residual energy was rendered safe. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure all potentially hazardous stored or residual energy that can expose employees to hazards are rendered safe before employees are allowed to perform maintenance and servicing work on machines.
