29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) An OKK MCV 660 Vertical Mill did not have adequate guarding to protect the operator from accessing pinch points and crushed by hazards created by the table movement. b) On or about 10/23/14, control keys were left in the machine panels allowing the machines to be run in setup mode, disabling safety features that keep employees out of the hazardous machine areas during operation. c) On or about 10/23/14, a Dean Smith & Grace Type 21 Manual Lathe was operated with the chuck guard open to allow an employee to catch shavings from a plastic part, exposing the employee to struck by and entanglement hazards from the rotating part and chuck. d) On or about 10/23/14, interlocks on the DOOSAN PUMA V550M VTL Cell machines L11 and L15 were not functioning, allowing the machines to be operated with the doors open exposing employees to rotating parts, flying chips, and cutting fluid.
