29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:    a) On or about 10/22/14, in the Gold Toe Store #350 at the Fashion Outlets of Niagara Falls, 1900 Military Rd., Niagara Falls, NY:   A Workforce 2-Light Portable Work Light Stand was connected to 120-volt ac power via an extension cord. The plug on the cord of the 2-Light Portable Work Light Stand lacked a grounding prong.    NO ABATEMENT CERTIFICATION REQUIRED
