29 CFR 1910.119(h)(3)(ii): The contract employer did not assure  that each contract employee was instructed in the known fire, explosion, or toxic release hazards related to his/her job and the process, and the applicable provisions of the emergency action plan.   On October 12, 2014, in the Chlor-Alkali Unit at Formosa Plastics Corporation, Texas, during turnaround activities, employees were not provided adequate training on the possible hazards they could encounter when bolts securing the pressure control valve failed exposing employees to a hydrochloric vapor cloud.
