29 CFR 1910.119(h)(3)(ii): The contract employer did not assure that each contract employee was instructed in the known fire, explosion, or toxic release hazards related to his/her job and the process, and the applicable provisions of the emergency action plan. On October 12, 2014, in the Chlor-Alkali Unit, during turnaround activities, employees were provided inadequate training on the potential hazards and they would encounter, such as, but not limited to, when bolts securing a pressure control valve failed and allowed hydrochloric acid to discharge into the atmosphere exposing employees to a hydrochloric vapor cloud.
