29 CFR 1910.119(h)(3)(ii): The contract employer did not assure that each contract employee was instructed in the known fire, explosion, or toxic release hazards related to his/her job and the process, and the applicable provisions of the emergency action plan. On October 12, 2014, at the Formosa Chlor-Alkali 500 area, during turnaround activities, employees were not provided adequate training on the possible hazards they would encounter when bolts securing the pressure control valve failed exposing employees to a hydrochloric vapor cloud.
