29 CFR 1926.451(e)(1): Where scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers, stairway-type ladders, ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used for access: Jobsite: Employees were exposed to falls while climbing the sides of two different types of tubular welded scaffolding not designed for climbing.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe-boards, safety net systems or personal fall arrest systems: Jobsite: Employee working on a two story house roof top exposed to falls greater than 30 feet with no fall protection. The Samuel Miller Mason Contractor was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 315572024, citation number 1, item number 1 and was affirmed as a final order on July 29.2011, with respect to a workplace located at 38 Pleasant Street Brunswick, Me 04011.
