29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    An employee was observed climbing the crossbar of a Tubular Welded Frame Scaffold that was not equipped with a ladder to gain access to the working platform. Employee's were expose to a fall hazard approximately 17' to the ground below.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:    Employees were performing plastering operations at the an open end of a tubular welded frame scaffold while not using any type of fall protection system. Potential fall height was measured to be 17 feet to the ground below.

29 CFR 1926.452(c)(3): Frames and panels shall be joined together vertically by coupling or stacking pins or equivalent means.    Employees were working from a Tubular Welded Frame Scaffold that was not erected in accordance with manufacture's specification using wire throughout the scaffolding system as a staking pin to join together the scaffold frames.
