29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition:    a.   On or about October 15, 2014, at the Reddick Florida substation, on top of transformer number 2, tools and spray lubricants were left scattered on the working surface, exposing employees to a trip hazard in the vicinity of exposed electrical conductors.

On or about October 13, 2014, at the Reddick Substation, employees assisting with Doble testing were not trained in safe work practices required for high voltage Doble transformer testing, and they had not received periodic reviews or updates.     b.   On or about October 15, 2014, at the Reddick Substation, employees assisting with Doble testing were not trained in safe work practices required for high voltage Doble transformer testing, and they had not received periodic reviews or updates.

29 CFR 1910.269(o)(4)(v): A ground was not placed on the high-voltage terminal and other exposed terminals when the test area was entered after equipment was deenergized:    a.   On or about October 13, 2014, at the Reddick Florida substation, in the Doble test area, the bushing terminals of the number 2 transformer were not grounded between test runs, exposing the Test Operator Assistant to an electric shock hazard when working in the test area.    b.   On or about October 15, 2014, at the Reddick Florida substation, in the Doble test area, the bushing terminals of the number 2 transformer were not grounded between test runs, exposing the Test Operator Assistant to an electric shock hazard when working in the test area.

On or about October 13, 2014 at the Reddick Substation, during Doble Testing, the Test Operator and the Person-in-charge did not conduct routine safety checks before each test series including, but not limited to verifying that required personal protective equipment was in use near the Doble test set leads, transformer bushings, and terminal jumper wires, exposing employees to a 10 kilovolt, 300 milliamp electrocution hazard.    b.   On or about October 15, 2014 at the Reddick Substation, during Doble Testing, the Test Operator and the Person-in-charge did not conduct routine safety checks before each test series including, but not limited to verify that required personal protective equipment was in use near the Doble test set leads, transformer bushings, and terminal jumper wires, exposing employees to a 10 kilovolt, 300 milliamp electrocution hazard.

29 CFR 1910.269(o)(5)(iv): A test observer, capable of implementing the immediate deenergizing of test circuits for safety purposes, was not present, and means for immediate deenergizing of the test circuit were not in effect when employees were present in the test area during testing:    a.   On or about October 15, 2014, at the Reddick, Florida Substation, in the Doble test area, during the excitation testing, a test observer had not present, and the Doble test extension safety interrupt buttons for immediate deenergizing of the Doble test set were defeated by taping one button and converting one to a footswitch, exposing employees to a 10,000-Volt, 0.3-Ampere electrocution hazard.

On or about October 13, 2014, at the Reddick Substation, during Doble Testing, the test area was not barricaded to control access to the test equipment and transformer, exposing employees to an electrocution hazard by accidental contact with energized electrical parts that were conducting 10 Kilovolts and 300 milliamps.    b.   On or about October 15, 2014, at the Reddick Substation, during Doble Testing, the test area was not barricaded to control access to the test equipment and transformer, exposing employees to an electrocution hazard by accidental contact with energized electrical parts that were conducting 10 Kilovolts and 300 milliamps.
