29 CFR 1910.146(g)(3): The employer did not provide training that established employee proficiency in the duties required by 29 CFR 1910.146, Permit-required confined spaces, and did not introduce new or revised procedures, as necessary, for compliance: a.) Hampton Veteran Affairs Medical Center: On or about 10/23/14, the employer did not train employees working in confined spaces of the company's revised permit/non-permit space policy in that vault 110B and steam tunnels were reclassified, as permit required confined spaces. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces: a.) Hampton Veteran Affairs Medical Center: On or about 10/09/14, the employer did not provide a sign to identify vault 110B has a permit-required space as a result employees were potentially exposed to an asphyxiation hazard. NOTE: A sign reading DANGER-PERMIT-REQUIRED CONFINED SPACE, DO NOT ENTER or using other similar language would satisfy the requirement for a sign. Veteran Affairs Medical Center was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910. 146(c)(2), which was contained in OSHA inspection number 313175937, citation number 1 item number 6b and was affirmed on 05/19/2010, with respect to a workplace located at One Veteran Drive, Minneapolis, MN 55417. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
