29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about 10/23/2014, one or more methods of machine guarding was not proved to protect operators and other employees in machine areas from:    a)  Struck by hazards caused by the rotating hotbox, reservoir tank and other projections of the Shellcore Machine #2 Model 400;    b)  Caught in hazards caused by the ingoing nip points on the tail pulley of Belt 5, the conveyor belt which transported sand from the Sand Muller to Elevator 6.    c)  Caught in hazards caused by the ingoing nip points on the head pulley of Belt 15, the conveyor belt which transported sand from Elevator 16.    d)  Struck by and caught in hazards caused by moving parts inside Elevators 6 and 16 which were accessible by latched doors which were not securely fastened or interlocked.    e)  Struck by and caught in hazards caused by moving parts inside the Sand Muller which was accessible by latched doors which were not securely fastened or interlocked.    f)  Amputation hazards caused by the unguarded unused portion of a horizontal band saw located in the Maintenance Department.    g)  Amputation hazards caused by the unguarded unused portion below the table of the metal cutting band saw located in the Pattern Shop.    h)  Struck by and caught in hazards caused by the moving parts of the Carver Rollover equipment in the Carver molding area.
