29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment: a- International Flavors & Fragrances, Inc., D & E Department, Dayton, NJ: The hazard assessment provided by the employer for personal protective equipment did not include all of the protectors needed to prevent employee exposure to hot water and/or steam in the event of a splash during trunion draining operations. On or about 10/21/14, an employee's lower body was not protected when splashed by hot water while performing trunion draining operations. Condition observed on or about 10/23/14.

The last written documentation that a monthly hoist chain inspection was conducted was in July, 2014. Condition noted on or about 11/4/14.
