29 CFR 1926.152(e)(4): The dispensing units shall be protected against collision damage. a. At the south side of the building where employees used a 100 gallon fuel tank to refill equipment such as, but not limited to aerial lifts and forklifts, the dispensing tank was not protected from damage, thereby exposing employees to a contact hazard.

29 CFR 1926.453(b)(2)(iv): Employees shall always stand firmly on the floor of the basket, and shall not sit or climb on the edge of the basket or use planks, ladders, or other devices for a work position. a. On the south side of the worksite where employees were connecting purlins from the elevated basket of an aerial lift, the employees were standing and climbing on the guardrails of the basket, thereby being exposed to a 28 foot fall hazard.

29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e). a. In the south side of the building where an employee was working from the elevated basket of an aerial lift, the employee's lanyard was not connected, thereby exposing employees to a 28 foot fall hazard. b. In the northwest corner of the building where employees were welding from the elevated basket of an aerial lift, employees lanyards were not connected, thereby exposing employees to a 32 foot fall hazard.
