29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:  On or about October 23, 2014 at worksite located at or near 6292 N Oak Trafficway in Gladstone, Missouri , employees were exposed to a struck by hazard in that the employee was working in area where there was a possible head injury from impact and was not protected by protective helmet.
