29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  On or about 10/22/2014, in the Office Inspection Laboratory, the employer allowed employees to use an 80% methanol solution in order to conduct aflatoxin testing of grain without ensuring that employees wear adequate eye protection.

29 CFR 1910.133(a)(3):  The employer did not ensure that each affected employee who wore prescription lenses while engaged in operations that involved eye hazards wore eye protection that incorporated the prescription in its design, or wore eye protection that could be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses.  On or about 10/22/2014, in the Office Inspection Laboratory, the employer allowed employees to conduct analysis for aflatoxin using an 80% methanol solution and did not provide eye protection that could be worn over the prescription lenses worn by employees.

29 CFR 1910.272(e)(1)(i): Current employees at least annually, and new employees prior to starting work in the grain handling facility were not trained in the general safety precautions, including recognition and preventive measures for the hazards related to dust accumulation:  On or about 10/22/2014, at the ADM Grain Company facility located at 12710 River Road in Destrehan, the employer allowed employees to enter the elevator in order to conduct daily grain quality inspections and work on the dock in order to observe grain movement without providing annual training in the recognition and preventive measures for the hazards related to dust accumulation.
