29 CFR 1915.14(b)(1)(iii): Hot work was permitted in the engine room or boiler spaces, for which a Marine Chemist or a Coast Guard authorized person certificate was not required under paragraph (a)(1)(i) of this section, before being tested by a competent person and determined to contain no concentrations of flammable vapors equal to or greater than 10 percent of the lower explosive limit. (a) On October 22, 2014 the inspection revealed that employees were potentially exposed to serious injury when performing hot work in an engine room that had not been tested by a shipyard competent person to ensure that explosive concentrations of flammable vapors were not present.

29 CFR 1915.503(a)(2)(ii): The employer permitted employees to perform hot work in areas that were not free of fire hazards or that had not been controlled by physical isolation, fire watches or other positive means. (a) On October 22, 2014 the inspection revealed that employees were potentially exposed to serious injury from fire hazards due to hot work being performed without a fire watch and within six feet of combustibles that were not physically isolated. Located at the fire boat under construction at 3255 N Hayden Island Drive in Portland, Oregon.

(a) On October 22, 2014 the inspection revealed that cable adjacent to the electrode attachment to the Miller AlumaPower arc welder had damaged outer insulation. The welding equipment was being used to perform work on the Portland Fire Boat at 3255 N Hayden Island Drive in Portland, OR.
