29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of (carbon monoxide) listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of (50 pmm):   a)      On or about October 23, 2014, for the employee working at Keybank National Association as a manager, the employer failed to ensure that the employees 8 hour time weighted average to carbon monoxide did not exceed 50 ppm.  Employee exposure to carbon monoxide reached levels of 93.7 ppm with a severity of 1.874 due to a malfunction with the facility's boiler system.     b)     On or about October 23, 2014, for the employee working at Keybank National Association as a teller, the employer failed to ensure that the employees 8 hour time weighted average to carbon monoxide did not exceed 50 ppm.  Employee exposure to carbon monoxide reached levels of 69.1 ppm with a severity of 1.382  due to a malfunction with the facility's boiler system.      c)      On or about October 23, 2014, for the employee working at Keybank National Association as a teller, the employer failed to ensure that the employees 8 hour time weighted average to carbon monoxide did not exceed 50 ppm.  Employee exposure to carbon monoxide reached levels of 181.4 ppm with a severity of 3.628 due to a malfunction with the facility's boiler system.

On October 23, 2014,  KeyBank National Association located at 245 North Superior St. Toledo, Ohio, failed to implement adequate engineering controls to reduce employee exposure to carbon monoxide in that three employees working on the main floor of the bank were overexposed to carbon monoxide at an 8 hour time weighted average of 181.4 at the highest which exceeded the OSHA permissible exposure limit of 50 ppm by 362%.  Carbon monoxide originated from an improperly installed and operating boiler system.
