29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation. Coast Guard Lower Government Housing Project; on or about 20 October 2014 and times prior thereto; the employer did not ensure that a tubular welded frame scaffold was erected on footings that would not settle; or cause them to become displaced. Two instances were noted where scaffolds were erected without base plates and mud sills. Scaffolds were positioned on a grassy surface. This condition exposed employees to a fall and struck by hazards in the event that the scaffold collapsed.

29 CFR 1926.452(c)(2): All brace connections were not secured. Coast Guard Lower Government Housing Project; on or about 20 October 2014 and times prior thereto; the employer did not ensure that a tubular welded frame scaffold was braced by cross, horizontal or diagonal braces, or combination thereof, to ensure that the scaffold was plumb, level, and square. Two instances were noted where scaffolds were erected without braces. Scaffolds were positioned on a grassy surface. This condition exposed employees to fall hazard.
