Coast Guard Lower Government Housing Project; on or about 20 October 2014 and times prior thereto; the employer did not ensure that a tubular welded frame scaffold was erected on footings that would not settle; or cause them to become displaced. Two instances were noted where scaffolds were erected without base plates and mud sills. Scaffolds were positioned on a grassy surface. This condition exposed employees to a fall and struck by hazards in the event that the scaffold collapsed.
