ESTABLISHMENT:  The orange flexible extension cord, used to power a Milwaukee Miter Saw was not protected from accidental damage where it was concealed behind the saw, went through rolled paper, around the concrete block corner, over broken glass and then was plugged into the electrical outlet box.
