WORKSITE: The Employer did not utilize safe and acceptable means to determine the exact location of underground utilities and the water main that were known to be present in and around the excavating area.    On or about October 24, 2014, the crew, engaged in the above task was using the bucket of the excavator to look for the water main and resulted in striking and damaging a gas line.
