29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) On or about October 16, 2014, Alejandro Estrella Roofing, LLC. did not ensure that employees were protected from falls while working at heights greater than 6 feet. Employees of Alejandro Estrella Roofing, LLC., were exposed to fall hazards in excess of 9 feet while engaged in roofing activities on the roof of a commercial building with no fall protection in place. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

Employees of Alejandro Estrella Roofing, LLC. were observed using a ladder to access the roof of a commercial building. Ladder did not extend at least 3 ft above the landing platform and was not tied to the roof edge. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
