Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems. On 10/16/2014, workers from Revelle Roofing and Exteriors LLC were repairing a roof of a residential building located at 14 Busse Court, Wentzville MO. They were not utilizing guardrail systems, a safety net system, or personal fall arrest systems. The foreman and three employees were observed on the roof of the building without fall protection, exposing them to a fall to the ground below.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. On 10/16/2014, workers from Revelle Roofing and Exteriors LLC were repairing a roof of a residential building located at 14 Busse Court, Wentzville MO. They were using a ladder improperly to access the roof. The ladder side rails did not extend to at least three feet beyond the landing surface or edge of the roof.
