On, or about, November 14, 2014, the door interlock switches provided on Machines M18 and M19, two Mori Seiki Model SL-65 Turning Centers, had been bypassed.      3.  On, or about, January 8, 2015, Machine M56, a Supermax V2110B turning center was not provided with an interlock on the access door during the automatic cycle and the auger-type chip conveyor was not safeguarded either by the use of a constant pressure control, door interlock or physical barrier guard.     4.  On, or about, January 8, 2015, the door interlock switches provided on Machine M87 and Machine M88, two Puma 450 turning centers, had been bypassed.      E.
