29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is not fully planked: On or about October 15, 2014, at Lot 10, 915 Reserve Point Place, Suwanee, Georgia: An employee performing brick masonry work was exposed to a fall hazard of approximately 9.3-feet when he worked from the work platform of a tubular welded frame scaffold that was not fully planked. Rufino Bastida was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(b)(1), which was contained in OSHA inspection number 903018, citation number 1, item number 1, and was affirmed as a final order on June 12, 2013, with respect to a workplace located at 4537 Summersweet Drive, Marietta, Georgia.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access. On or about October 15, 2014, at Lot 10, 915 Reserve Point Place, Suwanee, Georgia: An employee performing brick masonry work was exposed to a fall hazard of approximately 9.3-feet when the employer did not install a ladder and the employee had to climb the scaffold frame. Rufino Bastida was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(e)(1), which was contained in OSHA inspection number 314641572, citation number 1, item number 2, and was affirmed as a final order on July 7, 2011, with respect to a workplace located at 4904 Sydney Lane, Marietta, Georgia. Rufino Bastida was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(e)(1), which was contained in OSHA inspection number 422163, citation number 1, item number 1, and was affirmed as a final order on August 16, 2012, with respect to a workplace located at 2580 Walden Estates Drive, Marietta, Georgia.
