29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   At the jobsite located 801 Courtwood Lane, Ballwin, Missouri on or about October 23, 2014, employee(s) were observed installing shingles on a 2-story residential roof without the use of fall protection.  The fall distance to the ground below was greater than 6 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  At the jobsite located 801 Courtwood Lane, Ballwin Missouri on or about October 23, 2014, employee(s) were not instructed how to recognize and correct fall hazards while working on the roof. Employee(s) were observed working on a 2-story residential roof at greater than 6 feet from the ground without the use of fall protection.

At the worksite located 801 Courtwood Lane, Ballwin Missouri on or about October 23, 2014, employees were observed  walking below the 2-story residential roof where material was being removed and tossed to the ground below without protective helmets to reduce danger of head injury from falling debris.
