29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):   (a) On or about October 16, 2014, Daniel W. Grinnan, LLC. did not ensure that the working platform on their fabricated frame scaffold was fully planked or decked between the front uprights and the guardrail supports. An employee of Daniel W. Grinnan, LLC. was observed pressure washing wood from the top of a scaffold platform that was not fully planked. This condition exposed the employee to fall hazards in excess of 9 feet. 29 CFR 1926.451(b)(5)(i): The end of a platform 10 feet or less in length extended over its support more than 12 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilevered end:   (a) On or about October 16, 2014, Daniel W. Grinnan, LLC. did not ensure that the end of a scaffold platform did not extend more than 12 inches over its support. An employee of Daniel W. Grinnan, LLC. was observed pressure washing wood from the top of a scaffold platform where the wood planks extended more than 12 inches over their support. This condition exposed the employee to fall hazards in excess of 9 feet. An employee of Daniel W. Grinnan, LLC. was observed pressure washing wood from the top of a scaffold that was not level and square. This condition exposed the employee to fall hazards in excess of 9 feet. Grinnan, LLC.,as the controlling employer, did not ensure that employees of Alejandro Estrella Roofing, LLC., a subcontractor, were protected from falls while working at heights greater than 6 feet. Employees of Alejandro Estrella Roofing, LLC., were exposed to fall hazards in excess of 9 feet while engaged in roofing activities on the roof of a commercial building with no fall protection in place. Grinnan, LLC., as the controlling employer, did not ensure that portable ladders, used to access upper landing surfaces, extend at least 3 feet above the upper landing surface to which the ladders were used to gain access. Employees of Alejandro Estrella Roofing, LLC., a subcontractor, were observed using a ladder to access the roof of a commercial building. Ladder did not extend at least 3 ft above the landing platform and was not tied to the roof edge.
