29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  a) Building 3: On or about October 22, 2014, and at times prior, workers were exposed to falling 32 feet when the contractor did not implement and/or enforce an effective inspection program at the work site.    Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches, whichever is less, were not equipped with a stairrail system along each unprotected side or edge:    a) Trail Ridge, Bldg 3: On or about October 22, 2014, and at times prior, Cannon Construction, LLC, did not protect their own employee, and as a controlling employer, did not ensure that 719designs by levi, LLC, protected their employees from falling from a stairwell spanning the second to third floor when used to access a ladder to the roof.
