29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. (a)Concrete Frame Associates, LLC @ 1000 N. Speer Blvd., Denver, CO 80204: On and before October 8th, 2014 the exposing employer did not ensure employees working on the 7th floor near a floor hole used for access and egress to the 6th floor, were protected from falling through the hole. The hole was more than 6 feet above a lower level and the employer did not provide an adequate guardrail system around the hole to ensure employees would not inadvertently fall to the lower level. This condition exposed employees to a fall hazard greater that 6 feet in height.
