29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation: a) 1425 Morris St, Philadelphia, PA - The employer did not install all the baseplates on the erected fabricated frame scaffold thus exposing the employees to an unstable scaffold and scaffold collapse hazard. This condition was observed on or about October 17, 2014.

29 CFR 1926.451(e)(1): Where scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers or stairway-type ladders was not used for access: a) 1425 Morris St, Philadelphia, PA - The employer did not provide safe access for the employees to reach the working level of the scaffold. The employee was working from a fabricated frame scaffold platform, approximately 20 feet above the ground. This hazard exposed the employees to a fall hazard. This condition was observed on or about October 17, 2014.
