29 CFR 1926.451(e)(8): Direct access to or from another surface was used when the scaffold was more than 14 inches (36 cm) horizontally and/or 24 inches (61 cm) vertically from the other surface: On or About October 27, 2014 Location 354 Broadway, New York, NY, 6th Floor roof top, a). An Employee was working approximately 30 feet above the street level, from a 2 point suspended scaffold system located on the front (west) side of the building, and stepped out of the scaffold onto a roof top, on the north side of the building more than 24 inches from the scaffold end, exposing himself to fall hazards.
