29 CFR 1926.452(k)(5): Scaffold platforms were bridged on to another. Employees engaged in roofing operations on a residential structure used a ladder jack scaffold along the roof edge for roofing work, fall protection and roof access and the scaffold platforms were bridged on to other platforms.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): Employees engaged in roofing operations on a residential structure with a 10/12 roof pitch were exposed to fall hazards between 10-13ft' and the employer had not provided adequate fall protection in the area of the roof where a truck had been backed up to the eve to provide materials and along the rake edge of the garage. Jeff Stupavsky dba Jeff Stupavsky Roofing & Restoration was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 752561, citation number 1, item number 2 and was affirmed as a final order on 03/27/2013, with respect to a workplace located at 119 Knollwood, Quincy, IL 62301.
