29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:  Location:   250 Whisconier Road, Brookfield, CT 06804  The 9 in 12 pitched roof with a 17 foot 9 inch eave to ground fall distance was not provided with a safe means of access and exit due to employees being required to utilize the ladder jack scaffold support ladders and climb over and around the scaffold platforms and scaffold platform supports.
