29 CFR 1926.20(b)(2):     Such programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.    On 10/20/2014, the general contractor was watching the sub-contractor install Styrofoam on the side of a commercial building located at 221 St. Charles Rock Road, Bridgeton MO.  The general contractor or controlling employer did not conduct an adequate inspection of the job site, materials, and equipment.  The following hazardous conditions existed:        a)  Three employees were not protected by helmets while working beneath another employee on scaffolding. The  employee at the top of the scaffolding was using a hand trowel.      b)   Three employees were working on scaffolding, which was not fully planked.  The platform units were not installed so that the space between adjacent units was no more than 1 inch wide.      c)   Three employees did not have ladders to access the scaffolding; they were climbing on the sides and crossbracing to access the upper levels.      d)   Three employees were using scaffolding, which did not have guardrail systems installed along all open sides and ends of platforms.
