29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. At this commercial construction site, employees were installing trusses 10 feet above ground level. The employees were not protected from falling by the use of a guardrail system, safety net system, or personal fall arrest system.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. At this commercial construction site, flexible cords were used without strain relief. A flexible cord used to energize a circular saw had its outer insulation separated from the female plug.
