29 CFR 1910.119(l)(1): The employer did not establish or implement written procedures to manage changes to process chemicals, technology, equipment, and procedures, and changes to facilities that affect a covered process:    (a) At Diodes Fabtech Inc. on or about October27, 2014 did not develop and implement procedures to manage changes to the TCS covered process.  Changes to the process were made including changing of piping, adding monitoring equipment, and changing two valves.  A strap was installed in each of the cabinets used for dispensing the TCS with the intent of it being used to prevent movement of the drum of TCS.  No management of change had been done to determine if the non use of the safety strap created any hazard.      Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose. The same area contained numerous chemicals, some of which were highly corrosive, some pyrogenic, some flammable, some explosive, and some toxic.  The hazards of all of these gases and their potential for being released during an emergency event had not been fully considered in selecting areas for sheltering in place and/or evacuation.
