29 CFR 1910.178(l)(3)(i)(H): The employer did not evaluate employees knowledge and/or understanding of the powered industrial truck capacity and/or capability: On or about October 28, 2014, employees dismantling concrete beam forms were exposed to struck-by and crushing injuries. Employees were not trained to determine the weights of forms when multiple sections were bolted together which could overload the forklift capacity.
