29 CFR 1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who was on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems: On or about October 28, 2014, and at times prior thereto, at 1374 George Dieter Dr., an employee was observed conducting welding operations at the front and side edges of the metal roof without proper fall protection and was exposed to a fall hazard of greater than 15-feet.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high: On or about October 28, 2014, and at times prior thereto, at 1374 George Dieter Dr., the employer did not install a mid-rail between the top edge of the guardrail system and the walking/working surface, employees were exposed to fall hazards of greater than 15-feet.
