29 CFR 1926.453(b)(2)(v): A lanyard was not attached to the boom or basket when working from an aerial lift: On or about October 28, 2014, and at times prior thereto, at 106 Sunland Park Ave., an employee was observed working off a JLG T350 Tow-Pro Boom Lift without any form of fall protection and was exposed to a fall hazard of greater than six-feet.

29 CFR 1926.454(a): The employer did not have each employee who performed work on a scaffold be trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used, and to understand the procedures to control or minimize those hazards.
