The employer did not provide head protection for the employees to wear, thus exposing the employees to struck-by injuries from tools, mortar and debris. This condition was observed on or about October21, 2014.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contained or deflected the falling objects:  a) 1638 Morris St, Philadelphia, PA - The employees working in areas under the work platform of the scaffold were not provided additional protection from falling tools, debris and other objects. This hazard exposed the employees to struck-by injuries. This condition was observed on or about October 21, 2014.

29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports:    a) 1638 Morris St, Philadelphia, PA - The employees were working off of a fabricated frame scaffold approximately 14 feet long, 33 feet high and 5 feet wide which was not fully planked or decked on all working levels between the front uprights and the guardrail supports. This hazard exposed the employees to a fall hazard ranging from approximately 13 to 33 feet. This condition was observed on October 21, 2014
