29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:     a) 13 Niblick Street, Point Pleasant, NJ: Employees were allowed to use a ladder that was not secured and did not extend 3 feet above the working surface to allow for safe access on and off the roof used to access the pump jack scaffolding.     Condition observed on or about October 28, 2014.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   a)13 Niblick Street, Point Pleasant Beach, NJ: Employees were allowed to perform siding/window trim operations  approximately 15' from the ground below on a pump jack scaffold without fall protection.  Condition occurred on October 28, 2014.  Angor Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 313983470, citation number 1, item number 1 and was affirmed as a final order on 07/14/2011, with respect to a workplace located at 5000 Cedar Village Blvd. East Brunswick, NJ 08816.
