On 10/20/2014, workers from Architectural Sheet Metal Systems, Inc. were installing flashing on a leading edge of a roof of a commercial building located 11201 St. Charles Rock Road, Bridgeton, MO.  The foremen and another employee were working approximately sixteen feet above the ground without the proper use of guardrail systems, safety net systems, or personal fall arrest systems.
