29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a. During roofing work on a two story apartment building at 217 Sycamore, on 10-28-14, not all employees were connected to a lanyard/lifeline.   Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used.  a. During roofing work on a two story apartment building at 217 Sycamore, on 10-28-14, where training certification (documentation) did not exist for the fall hazards and use of fall protection at this job site; there was also indication that employees did not understand that re-positioning was not a reason for being connected at all times.   Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

During roofing work on a two story apartment building at 217 Sycamore, on 10-28-14, training certification (documentation) did not exist for the fall hazards and use of fall protection at this job site.   Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).
