Second Floor/First Kennel Room/four receptacle power outlet box - On or about October 30, 2014, the employer was utilizing an outlet box that showed two of its receptacles were damaged, bent, and poorly align with its metal cover/plate. Employees were thereby exposed to electric hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws. a. Second Floor/1st and 2nd Kennel Rooms - On or about October 30, 2014, a flexible electrical cord with its outer insulation pulled away from its rubber plug was being used to energized kitchen appliances. Employees were thereby exposed to electrical hazards.
