29 CFR 1926.21(b)(6)(i): Employees required to enter into confined or enclosed spaces were not instructed as to the nature of the hazards involved, the necessary precautions to be taken, and in the use of protective and emergency equipment required: a) Employees were required to enter and work inside a concrete valve vault manhole without being provided with safety instructions to recognize and control the hazards associated with entering and working in confined spaces. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c): a) Employees were not protected from the hazard of cave-ins by adequate shoring, sloping, or other protective means while working in a trench having a depth range of approximately 6 to 7 feet. Scott Ward Underground Contractors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1) which was contained in OSHA inspection number 315512152, citation number 1, item number 1, and was affirmed as a final order on August 2, 2011, with respect to a worksite located at 1047 Tower Road, Winnetka, Illinois. No abatement certification of documentation required for this violation.
