29 CFR 1926.451(b)(10): Scaffold components manufactured by different manufacturers were modified in order to intermix them and a competent person did not determine that the resulting scaffold was structurally sound:     a)  318 1st Ave Manasquan, NJ:  On or about 10/27/2014, two employees of Two Brothers Renovations were exposed to fall hazards due to intermixing of pump jack scaffold components. An extension ladder, laid flat, was used as a walking surface and a competent person did not determine that the resulting scaffold was structurally sound.     NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.     a)  318 1st Ave Manasquan, NJ:  On or about 10/27/2014, two employees of Two Brothers Renovations were installing siding from a pump jack scaffold.  The employees were not protected by guard rails, end stops, PFAS or any other form of fall protection and as such were exposed to a fall hazard of greater than 25 feet to the ground below.        NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
