29 CFR 1910.303(g)(2)(i):  Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):          a)    In the Dome Shop, near track 25, the 110 volt electrical panel breaker box, the block wall below had been damaged and the electrical wiring was exposed.  b)    In the Inspection/Supervisor Office, south section, a 110 volt electrical receptacle was pulled out away from the wall.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:     a)            In the Inspection/Supervisor Office, south end, on the 110 volt electrical Relocatable Power Tap, the ground circuit was not complete.     b)            In the Inspection/Supervisors Office, north end, on the 110 volt electrical receptacle that was part of the light switch, the ground circuit was not complete.     c)            In the Tool Room, east wall, on the 110 volt electrical receptacle, the ground circuit was not complete.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions:           a)    Outside Area, by the Lift Station, on the 480 volt electrical panel, the conduit that contained the electrical wiring was separated from breaker box, allowing strain to be placed upon the wiring that can cause abrasions in the insulation.     b)    In the Blast Bag House, the 230/460 volt motor on the dust collector, the conduit which contained the electrical wiring was separated from the motor box, allowing strain to be placed upon the wiring that can cause abrasions in the insulation.
